Requirements:   a PT degree from an accredited school; license to practice in California; one year of clinical experience; a demonstration of good verbal and written communication and organizational skills; a current CPR certification; a willingness to visit the service area of the agency; and suitable means of transportation to visit the patients.

Responsibilities:  A Physical Therapist (PT) performs the initial evaluation and develops the treatment plan in the area of mobility, functional status and rehabilitation.  During the initial evaluation s/he reviews the medical history and the patient's prior level of function (PLOF) / baseline mobility.  The PT determines the patient's needs and current level of function by assessing areas such as muscle strength, range of motion (ROM), and mobility.  The PT also determines the rehabilitative potential (the need and appropriateness for therapy); involves the patient (or family/caregiver) in establishing the functional goals; and refers other disciplines as needed.
